Thermage FLX in Seoul: How It Works, Pain, Recovery and Who It's For

Cheongdam RETOO ClinicWritten by Medical Director Dr. Kim Jae-rim

Thermage FLX is a fourth-generation monopolar radiofrequency device that heats the dermis from within while cooling protects the skin's surface. Cheongdam RETOO Clinic explains how the heat is made, what the Total Tip and Eye Tip are for, what AccuREP™ does, how it compares with focused ultrasound, and what to expect for comfort, downtime and results.

What is Thermage FLX, and who is it for?

Maybe your jawline hasn't changed much, but your skin feels softer and less springy than it used to, or the fine lines around your eyes stay put long after you stop smiling. That is often the point when people start researching skin tightening, and Thermage FLX tends to come up early. Thermage® FLX is a fourth-generation monopolar radiofrequency (RF) device made by Solta Medical. It was cleared by the US FDA in 2017 (K170758) and approved by Korea's Ministry of Food and Drug Safety in 2018 (approval no. 18-245) for the non-invasive treatment of wrinkles on the face and around the eyes [5][6].

At Cheongdam RETOO Clinic, it is most often considered by patients whose skin has lost firmness, who are bothered by nasolabial folds or a double chin as the cheeks and jawline loosen, who have crepey skin around the eyes, or who simply want a natural-looking lift without surgery. "Sagging," though, can mean quite different things. Sometimes the skin itself has thinned and slackened. Sometimes the mid-cheek has lost volume, which makes the nasolabial folds look deeper. Sometimes extra fullness under the chin weighs down the lower face. Thermage works on the first: the firmness and fine lines of the skin itself. It does not restore lost volume.

How does radiofrequency heat the skin?

Radiofrequency is an electrical current that switches direction extremely fast. Thermage FLX is monopolar, meaning the current travels between the treatment tip on your skin and a grounding pad placed on your body. Tissue resists that current, and the resistance produces heat, much as the coil in an electric heater warms up. So rather than building up at one spot, the heat spreads across the dermis and the tissue beneath it, along the current's path [5].

RF current flowing between the tip and the grounding pad meets the tissue's resistance and turns into heat, while cooling keeps the epidermis protected and the heat stays in the dermis.

Heat that reached the surface would burn, so the device cools the skin while it delivers heat to the deeper dermis, a principle the manufacturer calls the Reverse Thermal Gradient [5]. The outside stays cool; the inside warms. Heated dermal collagen denatures (its structure loosens) and contracts, and as that collagen is rebuilt, wrinkles improve [5].

The Reverse Thermal Gradient: cooling pulses chill the epidermis while RF heat stays in the dermis.

Both stages have been seen under the microscope. When researchers examined RF-treated skin with an electron microscope, collagen fibers looked thicker with blurred edges immediately after treatment, a sign of contraction. Later, expression of the type I collagen gene rose, pointing to new collagen production [1]. Put simply, the heat is delivered on treatment day, but the skin keeps changing well afterwards.

Which Thermage FLX tips are used, and what does AccuREP™ do?

Thermage FLX uses a single handpiece with a disposable tip. The Total Tip 4.0cm² (Purple Tip) treats the face, neck and body and comes in 300-shot and 600-shot versions. The Eye Tip 0.25cm² (Green Tip) is made for the eye area, where skin is thinner than elsewhere on the face, and comes in 225-shot and 450-shot versions [5]. Each tip is a single-use medical device locked to one patient; once its shots are used up, it cannot be used again [5].

AccuREP™ is an algorithm that reads the skin's electrical resistance (impedance) with every shot and adjusts the RF output automatically [6]. Skin thickness and hydration vary across the face, so a fixed setting would deliver more energy in some places and less in others. Recalibrating shot by shot lets each area receive the right amount [5]. The 4.0cm² tip is also 33% larger than the tip on the third-generation Thermage CPT, which cuts treatment time by about 25%, and the handpiece's vibration and cooling pulses help keep you comfortable [5].

Thermage vs. Ultherapy: what's the difference?

Thermage is often weighed against focused ultrasound lifting such as Ultherapy Prime, but the two heat the skin in different ways. Focused ultrasound concentrates energy at a single point at a set depth, creating tiny thermal coagulation points. Thermage FLX uses RF current to warm tissue over a broad area. Think of one as placing dots and the other as warming a whole surface.

MethodHow heat is createdMain concerns
Thermage FLX (monopolar RF)Resistive heat warms the dermis broadly · Epidermis is cooledSkin firmness · Fine lines · Eye area
Focused ultrasound (Ultherapy Prime)Ultrasound focused to points to form thermal coagulation pointsDeeper-layer sagging · Jawline
How Thermage FLX and focused ultrasound lifting deliver heat differently

That's why "which one is stronger?" isn't the right question. Skin that has slipped below the jawline and thin, finely lined skin around the eyes need heat at different depths and over different areas. An expert consensus on fourth-generation monopolar RF likewise concludes that choosing the right patient and the right technique is what shapes the outcome [4]. When deeper sagging and loss of firmness show up together, one option at Cheongdam RETOO Clinic is Ultherti, which combines both devices on the same day, each working at a different depth.

Does Thermage FLX hurt, and when will I see results?

You'll feel a quick burst of heat each time the tip touches the skin, followed by a cooling sensation [6]. The manufacturer recommends working with your provider to keep the energy at level 2, described as "a little hot but tolerable" [5]. A survey of 5,700 treatments across many practices found that multiple passes at lower energy, adjusted to the heat the patient reports, led to fewer pain complaints and higher satisfaction than the older single high-energy pass [3]. So speak up about how the heat feels; it helps your provider set the energy.

Redness fades within 24 hours, and any mild swelling settles within a few days [6]. There's no real downtime, so you can get back to your day. For about a week, skip saunas, Korean bathhouses (jjimjilbang), strenuous workouts, heavy drinking, smoking and peeling products, and keep up with sunscreen and moisturizer [5]. Thermage cannot be used if you have an electrical implant such as a pacemaker, and treatment over dermal filler or during pregnancy or breastfeeding has not been studied [6].

Results don't arrive on a single day. Some tightening shows right away, the change builds over 2–6 months as collagen is rebuilt, and it typically lasts 1–2 years depending on your skin and how quickly you age [6]. In a multicenter study of the eye area, wrinkle scores and brow position kept changing over the 6 months after a single treatment [2]. Seeing little difference the next morning doesn't mean it hasn't worked, and the tightness you feel right afterwards isn't the final result either.

Can I fit Thermage FLX into a short trip to Seoul?

If you're traveling from abroad, plan around the recovery timeline above. Redness usually fades within a day, swelling within a few days, and there's no real downtime. What matters more is the following week: no saunas, jjimjilbang, hard workouts, heavy drinking, smoking or peeling products, plus diligent sun protection. If your trip includes spa days or long hours outdoors, factor that in when you pick your treatment date.

It also helps to know what you'll see before you fly home. The immediate tightening is only the first stage; the main change builds over 2–6 months, so most of it will show after you're back. Taking photos in the same spot, light and expression makes later comparison easier. How long to stay in Seoul and when to follow up are decided at your consultation, based on your treatment and how your skin recovers.

Can I get an assessment from photos before I visit?

Photos are a good way to show which area bothers you, such as your eyes, cheeks or jawline, and to discuss a general direction. What a photo can't show is skin thickness, how much firmness you still have, or whether the issue is laxity, volume loss or deeper sagging. Those questions, along with the tip, shot count and treatment area, are settled at an in-person consultation that includes the 3D Precision Skin Diagnosis System described below. Please send photos of the area of concern only, and never photos of your passport or other ID.

How does Cheongdam RETOO Clinic approach Thermage FLX?

Loss of firmness looks different on every face, both in the skin and in the underlying structure, so Cheongdam RETOO Clinic doesn't run the same device the same way for everyone. Before treatment, the 3D Precision Skin Diagnosis System takes images under four light sources and maps about 70 facial landmarks to assess wrinkles, pores, pigmentation and firmness. The Medical Director then plans your tip, shot count and energy distribution in a one-on-one consultation.

During treatment, the clinic's patented HeatView™ System uses a thermal imaging camera to show, in real time, how heat is reaching the skin. On top of AccuREP™'s shot-by-shot adjustments, the team can see where heat is falling short or running high as they work, which helps prevent unwanted reactions such as burns or hollowing in the cheeks. Only genuine devices and tips are used, with the full shot count.

Picking a well-known device matters less than knowing what you actually want to address in your own face. If you're considering Thermage, start your consultation by finding out whether your concern comes from skin firmness, volume or deeper sagging.

Frequently asked questions

How many shots of Thermage FLX do I need?

The Total Tip comes in 300-shot and 600-shot versions and the Eye Tip in 225-shot and 450-shot versions [5]. Your shot count is set after assessing the area of concern and your skin thickness, and more shots aren't better for everyone.

How long does a Thermage FLX session take?

The 4.0cm² tip makes treatment about 25% faster than the previous generation, so the treatment itself takes around 30 minutes [5]. Allow extra time for your skin assessment and numbing cream.

How long do Thermage FLX results last?

Changes build over 2–6 months and last 1–2 years depending on your skin and how you age [6]. When to repeat is decided at a follow-up consultation based on your progress.

Can I have Thermage FLX during a short trip to Seoul?

Redness usually fades within 24 hours and mild swelling within a few days, with no real downtime, but saunas, hard workouts, heavy drinking and peeling products are off-limits for about a week. The main changes appear over 2–6 months, so how long to stay and when to follow up are decided at your consultation, based on your treatment and recovery.
